Wellbeing Care Series describes the principles and evidenced-based practices of population health management, as it relates to wellbeing care and preventive services. The course will use the latest research and data to instruct the public health challenges that are barriers to wellness. It will include understanding the seven priorities and key areas of focus for clinicians when instructing patients on a healthy lifestyle. Understanding the role of population health management within the practice’s patient population and protocols for preventive and chronic care services is discussed.
The course includes examples of important patient information and clinical data collection steps towards understanding the practice’s patient population.
